This project is a partnership between Serefin Health (service provider), Grey Bruce Ontario Health Team (stakeholder engagement and referrals) and Brightshores Health System (referrals; evaluation by the Office of Research and Innovation).

The MyCareSteps team acknowledges that Grey Bruce is located on the traditional territories of the Saugeen Ojibway Nation, including the Chippewas of Nawash Unceded First Nation and the Chippewas of Saugeen First Nation, and is home to Métis communities across the region.
